Part-time Dental Hygienist

Medical Director of Population Health

The Medical Director will provide direct patient care 3.5 days per week with 1.5 days administrative time.The Medical Director provides clinical and strategic leadership and direction to population health initiatives. The Medical Director will develop and execute strategies to improve patient care and meet the clinical objectives of all population health and risk-sharing arrangements. Collaborates with operational and finance leaders across the organization to enhance the delivery of care, adapt “best practices,” and develops and implements value-based financial incentive models, including upside gain-sharing arrangements, quality improvement incentive programs, governmental incentive programs, and other third party pay-for-performance programs. Population Health initiatives include but are not limited to quality improvement, risk score accuracy, government programs, KPIs, and improvement in ineffective utilization. The Medical Director helps clinicians focus on models of care that advance the health of the community and the individual, control cost, and enhances reimbursement through value-based purchasing and shared risk opportunities. The Medical Director will need working knowledge of P4P programs and care management models (ACOs, medical homes). Experience and demonstrated successful leadership of case management, disease management and /or population health programs. Knowledge of the concept of risk and how the application of risk management theory can alter population health outcomes. Experience working with the application of data analytics and performance measurement tools for improving patient outcomes. Minimum of 5 years in a clinical administrative role within a primary care setting. At least 2 years’ experience with governance and management in an established ACO and/or directing value-based reimbursement models. Director of Population Health

The Director is responsible for designing and implementing a robust population health management program at Western North Carolina Community Health Services aimed at improving patient centric health outcomes. The DHP will lead development of population health priorities including streamlining/coordinating disease management and complex care management. Director, Integrated Behavioral Health (IBH)

The Director, Integrated Behavioral Health (IBH) oversees all aspects of clinical integration across Western North Carolina Community Health Services and is responsible for developing and guiding implementation of the IBH strategy and program as part of population health approach. The Director leads, manages and oversees a team of IBH clinicians in the provision of all integrated behavioral health services and is responsible for the sustainability of integrated behavioral health, as well as the outcomes associated with IBH performance standards. The Director will serve as the subject matter expert for behavioral health, responsible for overall strategy development, including standardized care management approach for members with behavioral health needs and coordination strategy with delegated behavioral health entities Ten years of clinical experience providing evidence-based therapeutic treatment services with Mental Health and/or Substance Use pediatric and adult populations and a working knowledge of public/private behavioral health/substance abuse treatment delivery systems. Five years’ experience in a supervisory role, supervising staff, programs, systems, and grants. This position is a combination of clinical and administrative role, which will consist of at least 3.5 days of direct patient care with 1.5 days of administrative time. Master’s Degree as a Licensed Clinical Social Worker (LCSW) in the state of North Carolina, or master’s degree in psychology or counseling is required. Advanced Practice Clinician for Primary Care (Mobile Unit)

Front Office Supervisor

The Front Office Supervisor provides direct supervision to the front office and call center staff and manages processes associated with patient check in/out, customer service, appointment scheduling, telephone call management, registration, co-pay collections, insurance verification, patient recall and end of day closure. S/he is responsible for creating efficient front office processes, allocating resources effectively to meet/exceed patient and internal customer needs.

  • Ensures that patients and visitors are greeted warmly with courtesy, friendliness and the highest degree of customer service and professionalism at all times.
  • Ensures that front office/call center staff support the needs of the providers and clinical team with a high degree of professionalism, courtesy, and responsiveness. Regularly seeks feedback from providers and clinical staff.
  • Continually observes and evaluates workflows, call volumes and patient throughput. Regularly devises changes in workflows and processes to increase efficiency and improve wait times and throughput.
  • Analyzes call volume highs and lows and adjusts staffing accordingly.
  • Recommends innovative approaches to streamline front office processes for efficiency and to provide a better patient experience.
  • Keeps up with industry standards related to efficient patient scheduling models, patient reminder systems, efficient check-in processes and queue management.
  • Develops and communicates written standards and protocols for all front office/call center functions (i.e., patient scheduling, check in/out, opening and closing procedures, insurance verification, etc).
  • Develops performance standards and metrics for key tasks of front desk/call center staff. Designs reports to monitor staff performance.
  • Coaches/mentors staff and develops written performance improvement plans when performance of staff does not meet expectations; monitors improvement and takes necessary action to ensure expected performance is achieved.
